What protected health information is

Protected health information (PHI) is information that identifies you and relates to your health, care, or payment for care — for example intake answers, session notes, diagnoses, and insurance details.

PHI is collected by the booking and telehealth platforms used by this practice — Headway, Rula, and Grow Therapy — when you register and attend sessions there. This website itself does not collect PHI and is not a HIPAA-certified system.

How information may be used

• Treatment: to provide counseling and, with your permission, coordinate with other providers involved in your care.
• Payment: to verify benefits and bill your health plan through the booking platform you use.
• Health care operations: for scheduling, quality review, supervision or consultation, and required record keeping.
• Required by law: limited disclosures such as suspected abuse or neglect of a child, elder, or dependent adult, or a serious threat of harm to you or someone else.

Other uses and disclosures generally require your written authorization, which you may revoke in writing at any time.

Your rights

• Ask to see or receive a copy of your record
• Request a correction to information you believe is inaccurate
• Request confidential communication through a specific phone number or email
• Request limits on what is shared, including with your health plan for self-pay care
• Receive an accounting of certain disclosures
• Receive a paper or electronic copy of this notice

Complaints

If you believe your privacy rights have been violated, you may contact the practice at admin@sarahjtherapy.com, the platform where you receive care, or the U.S. Department of Health and Human Services Office for Civil Rights. You will not be retaliated against for filing a complaint.
