How form submissions are used
Contact form submissions go to the practice admin inbox and are used for scheduling and answering your question. They are not used for advertising and are not sold or shared with third parties for marketing.
Please do not include sensitive health details in the form. It is not HIPAA-secure clinical intake.
This website is not a medical record
This website is not a HIPAA-covered electronic medical record. Clinical records, scheduling, insurance details, and session notes live on the platforms where you book and attend care: Headway, Rula, and Grow Therapy. Each platform maintains its own privacy practices and security controls.
Deleting a form submission
To request deletion of a message you sent through this site, email admin@sarahjtherapy.com and we will remove it from the inbox.
